A woman, ill with warm disease for some ten days, with robust fever that would not abate, worse at night, restless and agitated with intermittent delirious speech; mouth dry yet paradoxically not craving drink; faint macular rash dimly visible beneath the skin; tongue crimson and denuded; pulse thin and rapid.
温热之邪内陷心营,热灼营阴,神明受扰。属营分证。
Warm-heat pathogen sinking inward into the Heart's nutritive aspect, scorching nutritive yin, and disturbing the spirit-brightness. Nutritive-level pattern.
投清营汤清营透热,加竹茹以清心化痰。三剂后神清语正,斑疹消退;继以育阴之品调理,旬日而安。
Clear-the-Nutritive Decoction was given to clear the nutritive and vent heat, with Zhu Ru added to clear the Heart and transform phlegm. After three doses the spirit cleared and speech became rational and the rash receded; subsequent yin-nurturing regulation brought recovery within ten days.
叶氏曰:'入营犹可透热转气。'口干不甚渴饮,正是营热蒸腾津液上潮之征,与气分大渴引饮迥别。透热之关键在于金银花、连翘之轻清,引营分之邪复转气分而解。
Ye declared: 'Once heat enters the nutritive, one may still vent it back out to the qi level.' The dry mouth without marked thirst is precisely the sign of nutritive heat steaming fluids upward — quite different from the great thirst of qi-level heat. The pivot of venting heat lies in the light clearing action of Jin Yin Hua and Lian Qiao, which lead the nutritive pathogen back to the qi level for resolution.
