患者,妇,二十九岁。素多忧思,近因家事郁怒,两胁胀痛,胸闷善太息,月事不调,食欲不振,神疲倦怠,口燥咽干。舌淡红苔薄白,脉弦而虚。
A twenty-nine-year-old woman, prone to worry and brooding, recently angered by family troubles. She presents with distending pain in both flanks, chest oppression, frequent sighing, menstrual irregularity, poor appetite, weary spirit, and dry mouth and throat. Tongue pale-red with thin white coat; pulse wiry and empty.
肝郁血虚,脾弱失运,肝脾不和之证。
Liver constraint with blood deficiency and weak spleen failing to transport — a disharmony of liver and spleen.
服七剂,胁痛胸闷大减,情怀渐舒;再进半月,月事如期,食欲复常。丹溪云:'气血冲和,万病不生,一有怫郁,诸病生焉。'
After seven doses the flank pain and chest oppression eased greatly and her mood began to relax; another half-month brought menses back on schedule and normal appetite. As Danxi says: 'When qi and blood flow smoothly, ten thousand diseases cannot arise; the moment there is stagnation, all diseases spring up.'
逍遥散疏肝解郁而不伤正,养血健脾而不碍气。丹溪谓'郁者,结聚而不得发越也',故治郁在于调畅气机,非徒破气散结。
Xiao Yao San soothes the liver and dissolves constraint without injuring the upright, nourishes blood and strengthens spleen without obstructing qi. Danxi defined 'constraint' as 'that which clumps and cannot issue forth' — so treating constraint means smoothing the qi dynamic, not merely breaking and dispersing.
