患者,男,五十二岁,素嗜肥甘,体胖湿盛。近半年发作眩晕,每晕则头重如蒙,视物旋转,恶心呕吐痰涎,胸闷脘痞,食少神疲,头痛如裹。舌胖苔白腻,脉弦滑。
A fifty-two-year-old male, fond of rich fatty foods, of damp, heavy build. For the past six months he has had bouts of dizziness in which the head feels swathed and heavy, objects whirl, he retches phlegmy mucus, with chest oppression, epigastric stuffiness, poor appetite, weary spirit, and a wrapped sensation of headache. Tongue swollen with greasy white coat; pulse wiry and slippery.
脾虚生湿,聚湿成痰,风痰上扰清空,发为眩晕。
Spleen deficiency generates damp; the damp pools into phlegm; wind-phlegm harasses the clear empty space above, producing dizziness.
服五剂,眩晕发作次数减半,呕恶止;续服十剂,头重胸闷悉除,纳谷转香;嘱忌肥甘厚味,调摄半载而未再作。
After five doses the attacks were halved and the retching stopped; after ten more the head-heaviness and chest oppression were gone and appetite sweetened. With advice to abstain from rich greasy food and half a year of care, he had no further attacks.
程钟龄云:'眩晕者,中风之渐也。'无痰不作眩,无虚不作眩。治痰为标,健脾为本;天麻定风,为风痰眩晕要药。
Cheng Zhongling said: 'Dizziness is the forerunner of wind-stroke.' Without phlegm dizziness does not arise, and without deficiency it does not arise. Treating phlegm is the branch, strengthening the spleen is the root; Tian Ma settles wind and is the key herb for wind-phlegm dizziness.
