患者,女,二十八岁。外感七日,寒热往来,一日数作,胸胁苦满,心烦喜呕,默默不欲饮食,口苦咽干,目眩。舌苔薄白微黄,脉弦。
A twenty-eight-year-old female. Seven days into an external contraction she has alternating chills and fever several times a day, bitter fullness in the chest and hypochondrium, vexation with tendency to vomit, silent loss of appetite, bitter taste in mouth with dry throat, and dizziness. Tongue with thin white and slightly yellow coat; pulse wiry.
邪入少阳,枢机不利,胆胃不和,半表半里之证。
The evil has entered Shaoyang; the pivot mechanism is obstructed, gallbladder and stomach are in disharmony — a classical half-exterior, half-interior pattern.
服药一剂,寒热减半,胸胁苦满渐舒;三剂后往来寒热止,呕逆除,胃纳复;调理数日而痊。
After one dose the chills-and-fever were halved and the hypochondriac fullness began to ease; after three doses the alternating fever stopped, retching ceased, and appetite returned. A few more days of gentle care brought full recovery.
柴胡证但见一证便是,不必悉具。小柴胡和解少阳,为枢机之剂,切忌汗下。
For a Chai Hu presentation, even one of its cardinal signs suffices — not all need be present. Xiao Chai Hu Tang harmonises Shaoyang as the pivot; sweating and purging are strictly contraindicated.
