患者,男,三十二岁,冬月冒雪远行。归即恶寒发热,头痛身疼,腰痛骨节烦疼,无汗而喘,不渴。舌苔薄白,脉浮紧。
A thirty-two-year-old male who had trudged a long way through winter snow. On returning he developed aversion to cold with fever, headache and body aches, lumbar pain and vexing bone-joint pain, wheezing without sweat, and no thirst. Tongue with thin white coat; pulse floating and tight.
太阳伤寒,寒邪束表,卫闭营郁,肺气失宣。
Taiyang cold-damage: cold evil binds the exterior, wei is locked and ying is constrained, and lung qi loses its diffusing function.
一剂药下,温覆片刻,遍身漐漐汗出,寒热顿解,喘止身和。次日仅以稀粥将养而愈。
One dose was given; after a short period of warm covering a general light sweat broke out, the chills-and-fever suddenly resolved, wheezing stopped, and the body relaxed. The next day he needed only thin gruel for recovery.
无汗而喘、脉浮紧为麻黄汤的铁证。发汗峻剂,中病即止,汗出即停后服,勿令大汗亡阳。
Absence of sweat with wheezing and a floating-tight pulse are the ironclad markers for Ma Huang Tang. Being a vigorous diaphoretic, it should be stopped as soon as sweat appears — never push it to the point of profuse sweating and yang collapse.
