患者,男,四十八岁,儒吏,劳心过度,饮食失节。身热自汗,热在肌表,得劳则甚,倦怠懒言,气短乏力,头晕目眩,食少便溏,舌淡苔薄白,脉洪大而虚,重按无力。
A forty-eight-year-old male clerk, mentally overworked and irregular in meals. He presents with body heat and spontaneous sweating, heat felt at the surface and worsened by exertion, with lassitude, reluctance to speak, shortness of breath, dizziness, reduced appetite, and loose stool. Tongue pale with thin white coat; pulse surging and large but empty, forceless on deep pressure.
脾胃虚弱,中气下陷,阴火上冲,气虚发热之证。
Weakness of spleen and stomach with sinking of middle qi and upsurge of yin-fire — a pattern of qi-deficiency fever.
服五剂,身热渐退,汗止;十剂后倦怠改善,食欲大增;继以丸剂调治月余,诸症悉平,精神健旺如初。
After five doses the body heat gradually abated and sweating stopped; after ten doses fatigue improved and appetite returned markedly. A pill form was then used for over a month, and all symptoms resolved with vigour fully restored.
甘温除大热,非苦寒之热可比。脉洪大而按之无力是气虚发热之眼目,误用寒凉必坏中气。升麻、柴胡少用以升提清阳,乃东垣创格。
Sweet-warm herbs that dispel great heat: not a heat for bitter-cold to treat. A surging-large pulse that is forceless on pressure is the hallmark of qi-deficiency fever; bitter-cold error here will wreck the middle qi. Small doses of Sheng Ma and Chai Hu to lift clear yang are Li Dongyuan's signature innovation.
