某男,年四十,久病之后,面色萎黄,食少纳呆,食后脘腹胀满,大便溏薄日二三行,四肢倦怠乏力,舌淡苔白,脉濡缓。
A man of forty, convalescing from prolonged illness, with a withered sallow complexion, scant appetite and poor intake, epigastric and abdominal fullness after meals, loose thin stools two or three times daily, fatigue of the four limbs, pale tongue with a white coating, and a soggy moderate pulse.
脾胃虚弱,运化无权,中气不健,湿从内生。
Vacuity of the Spleen and Stomach with impaired transformation and conveyance; the middle qi fails to function, generating internal dampness.
以参苓白术散益气健脾、渗湿止泻,加陈皮以理气醒胃。连服十余剂,纳食渐增,便溏已止,精神亦振;继以丸剂缓图,两月康复。
Shen Ling Bai Zhu San was used to augment qi, fortify the Spleen, percolate dampness and arrest diarrhea, with Chen Pi added to rectify qi and awaken the Stomach. After more than ten doses appetite gradually increased, loose stools ceased, and the spirit revived; a subsequent pill form was used for gradual restoration, with full recovery in two months.
叶氏治脾胃分阴阳:'太阴湿土,得阳始运;阳明燥土,得阴自安。'此案脾阳不振,湿浊内停,故用参苓白术散甘淡渗湿,不用峻补刚燥之品。脾运一健,诸证自除。
Ye distinguished yin and yang in Spleen-Stomach treatment: 'The damp earth of Taiyin moves only when it receives yang; the dry earth of Yangming rests only when it receives yin.' Here Spleen yang is unrouse with damp turbidity retained internally, so Shen Ling Bai Zhu San is used to percolate dampness with sweet-bland herbs, avoiding forceful warming or harsh drying. Once Spleen conveyance is restored, all signs resolve of themselves.
