Fuzi (processed daughter root of Aconitum) contains highly toxic diester diterpene alkaloids (aconitine, mesaconitine, hypaconitine) plus the cardiotonic higenamine; processing hydrolyzes diester alkaloids to less toxic monoester and amine-alcohol forms. Pharmacology shows cardiotonic, anti-shock, analgesic and anti-inflammatory effects within a narrow therapeutic window.
Chinese Pharmacopoeia enforces strict upper limits on total diester alkaloid content; only processed forms (heishunpian, baifupian, paofupian) are permitted and must be decocted first and long.
