某妇,温病后期,热势已低,但身热夜甚,心烦不寐,口干咽燥,唇齿干枯,手足心热甚于手足背,舌绛无苔,脉虚大按之无力。
A woman in the late stage of warm disease: the heat had subsided somewhat but fever was worse at night, with vexation and insomnia, dry mouth and parched throat, withered lips and teeth, palms and soles hotter than their dorsa, a crimson denuded tongue, and a vacuous large pulse without strength upon pressure.
温邪深入下焦,耗伤肝肾真阴,邪少虚多。
Warm pathogen penetrating to the Lower Burner, exhausting true yin of Liver and Kidney; pathogen scant, vacuity predominant.
遵鞠通法以加减复脉汤(即炙甘草汤去参桂姜枣加白芍)咸寒甘润,填补真阴。服五剂神安寐稳,舌上津回;续服十余剂,精神渐复。
Following Wu Jutong's method, a Jia Jian Fu Mai Tang (Zhi Gan Cao Tang minus Ren Shen, Gui Zhi, Sheng Jiang, and Da Zao, with Bai Shao added) was used to provide salty-cold, sweet-moistening replenishment of true yin. After five doses the spirit settled and sleep steadied, and moisture returned to the tongue; a further ten-odd doses saw vigor gradually restored.
鞠通论下焦温病:'热邪深入,或在少阴,或在厥阴,均宜复脉。'去参、桂、姜、枣之温燥,加白芍以敛阴,易名'加减复脉汤',正是因证立方之变。虚热不可苦寒直折,宜甘润咸寒以滋其真阴。
On Lower Burner warm disease Wu Jutong stated: 'When heat pathogen penetrates deeply — whether in Shaoyin or Jueyin — Fu Mai Tang is appropriate in every case.' Removing the warming-drying Ren Shen, Gui Zhi, Sheng Jiang and Da Zao and adding Bai Shao to astringe yin, renaming it 'Modified Fu Mai Decoction,' is precisely formula-modification tailored to the pattern. Vacuity heat cannot be checked by bitter cold; it demands sweet-moistening and salty-cold nourishment of true yin.
