患者,男,三十四岁,嗜酒色,夜寐不安。腰膝酸软,头晕耳鸣,五心烦热,盗汗梦遗,咽干舌红少苔,脉细数。
A thirty-four-year-old male, indulgent in wine and intimacy, restless at night. He presents with sore weak low back and knees, dizziness with tinnitus, vexing heat in the five centres, night sweats and nocturnal emission, dry throat, red tongue with scanty coat, and a thin rapid pulse.
肾阴亏虚,水不制火,相火妄动,虚热内扰。
Kidney yin deficiency — water fails to restrain fire, ministerial fire stirs astray, and empty heat harasses within.
以六味丸常服,佐以知母、黄柏少量,月余盗汗止,梦遗减,腰膝转强;三月后五心烦热悉除,精神健旺。
Liu Wei Di Huang Wan was taken steadily with small additions of Zhi Mu and Huang Bai. After a month night sweats ceased and emissions diminished, with strengthening low back and knees; after three months the vexing heat in the five centres was entirely gone and vigour returned.
丹溪倡'阳常有余,阴常不足',治杂病以滋阴降火为要。六味地黄三补三泻,壮水之主以制阳光,非苦寒直折可比。
Danxi taught that 'yang is usually in excess and yin is usually deficient', treating miscellaneous disorders mainly by enriching yin and descending fire. Liu Wei Di Huang pairs three tonifiers with three drainers — 'strengthening the ruler of water to restrain the radiance of yang' — a very different approach from bluntly cutting heat with bitter-cold.
