An old gentleman past sixty, by nature quick to anger, with headaches and dizziness for several years, always worsened by vexation. His head feels bursting, tinnitus hisses like cicadas, face and cheekbones flush, he is vexed and sleeps little, with sore weak low back and knees, bitter mouth, and dry throat. Tongue red with scanty fluids; pulse wiry, thin, and rapid.
Liver-kidney yin deficiency: water fails to moisten wood, liver yang rises in excess and harasses the clear orifices above.
The master said: 'Water is depleted and wood exuberant — only enriching water to moisten wood will do.' Liu Wei Di Huang Wan with Tian Ma, Gou Teng, Ju Hua, and Bai Shao was taken steadily. After a month headache was greatly reduced, dizziness stopped, and sleep and appetite settled; slow-acting pills for another three months saw the symptoms all but gone.
Ye Tianshi framed dizziness as 'yang transforming into internal wind'. Enriching water to moisten wood, with Tian Ma and Gou Teng to subdue yang and extinguish wind, is the orthodox treatment for liver-kidney yin deficiency stirring wind.
