Diarrhea presents with increased frequency of bowel movements and loose or watery stools. The disease site is the intestines, but the key organ is the Spleen. Spleen vacuity with dampness predominance is the fundamental pathomechanism.
Key differentiation: Acute diarrhea is mostly repletion, primarily dampness; chronic diarrhea is mostly vacuity, primarily Spleen weakness. Clear watery stools indicate cold; yellow foul-smelling stools indicate heat. Pre-dawn diarrhea often indicates Kidney yang vacuity.
